Tracy Weber

Tracy Weber RN, BSN- High-Rish Disease Case Manager- Captive Health

Tracy joins Captive Health as a dedicated and compassionate nurse with extensive experience in patient care.  She received her bachelor’s degree in nursing from Regis University in Denver Colorado and has held a variety of nursing roles ranging from bedside to pediatrics to home health.  Her clinical training is complemented by expertise as a massage therapist, yoga teacher, and health coach. She is known for fostering holistic wellness through both traditional medical practices and complementary therapies. Tracy provides personalized care and support to individuals on their journey to optimal health and well-being. She is passionate about empowering patients to take an active role in their health through education, mindfulness, and therapeutic interventions.

As a Case Manager at Captive Health, she provides education, guidance and management with ongoing health issues. In addition, she maintains communication between clients and their primary care provider to help to fill in any gaps in care and improve overall health.  

In her personal time, she enjoys yoga, hiking and biking outdoors, and spending time with her family.
